Home Based Services
Home Based Care is important in providing health and support services, enabling post hospital care in the home environment, and the best possible quality of life in the home for disabled and frail aged community members. It is recognised that many people are happier and complete their recovery more quickly when in their own home environment.
It must be noted that patients are not released from hospital until their condition is considered suitable by their doctor. For information on any of these services please call 03 5154 6623 or 03 5154 6652.
The following services and staff provide cross-linked services, so the headings are only very broadly based:
District Nursing
This service provides across the board assistance to families and individuals, enabling frail, aged and those with disabilities to remain in their own home with the best possible level of independence and quality of life. The District Nurse operates from Monday to Friday and works in cooperation with the Home Based Services in preparing and delivering a range of services, such as assisting with medication and dressings.

Home & Community Care (HACC)
Home & Community Care services have a bank of trained staff to provide a wide range of home services to the frail, aged and those with disabilities in order to aid them in remaining in their own homes as long as possible. For example:
- Home Help (cleaning and other domestic duties)
- Home Maintenance (services related to basic home maintenance, eg; lawn mowing, installation of medical & physical aid equipment, basic repairs to doors, windows, etc, all aimed at keeping properties functional & in a safe condition
- Meals on Wheels (fresh and frozen meals provided for lunch and dinner)
- Personal Care (assistance with personal care, such as showering, dressing, shaving and grooming)
- Veteran's Home Care (basic home services provided to eligible veterans and families)
- Respite in the Home or respite in Orbost Regional Health either at Lochiel House Hostel or in the Hollands Wing Acute Ward
- People with Disabilities

Palliative Care in the Home
Our Palliative Care Service is made up of a variety of people and service types, all designed to assist you if and when you need them.
Your Palliative Care Nurse and your Doctor will be your main contact people. They can refer you to other services when and if it is necessary, or alternatively, you can contact these services directly. Services can be either individually accessed or a "package of care" developed to meet the needs of each person, their family and their carers.
A range of services are available that provide specialist and generalist support with the aim of improving quality of life.
Available Services include:
- Specialist Palliative Care Nursing
- Social Workers
- Personal and Family Counselling
- Medical Equipment for Loan or Hire
- General Practitioners
- Home Care
- Grief and Loss Counselling Trained Volunteers
- Respite Care
- Meals on Wheels
- Carer Support Group & Bereaved Partners Group
- Support is provided to families and carers based on individual needs.

Post Acute Care (PAC)
Post Acute Care services are provided to patients when they leave hospital if some further minor nursing care (eg; dressings), home or personal care is required.
A benefit of this service is a decrease in the demand for limited acute beds, leaving more available for emergency care.

Volunteer Network
As with many other organisations, ORH could not run without the valuable assistance of caring volunteers. Volunteers are trained in their chosen fields, including:
- Provision of transport to appointments
- In-home respite care
- Meals on Wheels
- Adult Literacy program
